If you need to add a dependent who is not currently enrolled on your plan please complete the SISC membership change form. This will automatically enroll your dependent to your Medical, Prescription, Dental, Cancer, Vision and Life Insurance plans. You must include a copy of your spouse’s marriage certificate and/or child’s birth certificate along with their social security number. NEW REQUIREMENT: The first page of the most current Federal Tax Return AND a copy of the marriage certificate are required for proof of spouse eligibility.
If you would like to make any changes to your current dental plan please click on the appropriate link below, print and complete appropriate form. To review dental insurance plan options, click on the Dental Insurance link on the right side of this page. Don’t forget to bring all completed enrollment forms into Health Benefits by August 25th!
If you are an employee working less than 4 hours per day or a substitute employee, a basic Health Plan option will be offered during Open Enrollment. Click on the “Bronze Plan” tab to the right.
